L4-L5 and L5-S1 — Why These Discs Are Most Affected
The L4-L5 and L5-S1 discs in the lower lumbar spine bear the most weight and the most movement, which makes them the most common sites for disc bulge and herniation. They sit close to the sciatic nerve roots — which is why disc problems at these levels cause the characteristic shooting pain down the leg.
L4-L5 disc bulge typically causes pain radiating to the back of the thigh and may cause weakness when lifting the big toe. L5-S1 disc problems often cause pain extending to the calf and outer foot, with possible weakness in the ankle.

The Ayurvedic Perspective
In Ayurveda, disc problems are understood as a manifestation of aggravated Vata dosha affecting the Asthi (bone) and Majja (marrow/nerve) dhatus. The condition is referred to as Gridhrasi when sciatica is present. Signs & Symptoms of a Disc Bulge

Localized back or neck pain: A deep, persistent ache that often worsens with sitting, bending, coughing, or sneezing.
Radiating pain (sciatica): Sharp, shooting pain travelling from the lower back into the buttock, thigh, calf, or foot — the hallmark of an L4-L5 or L5-S1 bulge.
Numbness or tingling: A "pins and needles" sensation along the path of the affected nerve, in the leg, foot, arm, or hand.
Muscle weakness: Difficulty lifting the foot or big toe, a feeling that the leg may "give way", or weakness when gripping with the hand.
Stiffness and reduced movement: Trouble standing straight, bending, or turning, often worse first thing in the morning.
Lumbar (lower back) disc bulge tends to cause leg-dominant symptoms — sciatica, calf or foot numbness, and weakness. Cervical (neck) disc bulge causes neck pain with radiating symptoms into the shoulder, arm, or hand. Identifying which level is involved guides the treatment plan.
When to seek care urgently
Loss of bladder or bowel control, numbness around the inner thighs or groin (saddle area), or rapidly worsening weakness in both legs can signal nerve-root compression that needs immediate medical attention. These red-flag symptoms are uncommon, but if you have them, seek emergency care before considering any elective treatment.
How a Disc Bulge Is Diagnosed
An accurate diagnosis tells us exactly which disc is involved, how far it has bulged, and whether it is compressing a nerve — all of which shape your treatment plan. Diagnosis usually combines:
Clinical examination: Our doctors assess your posture, range of motion, reflexes, muscle strength, and sensation, plus nerve-tension tests such as the straight-leg raise to locate the affected level.
MRI scan (the gold standard): An MRI shows the disc, the spinal nerves, and the degree of compression in detail. We ask every patient to share a recent MRI so we can plan treatment precisely.
X-ray or CT (when needed): Useful for assessing bone alignment, disc-space narrowing, or arthritis alongside the disc problem.

MRI reports use specific terms for how far disc material has moved. Knowing them helps you understand the severity:
Disc bulge
The disc extends beyond its normal boundary but the outer wall is intact. The mildest stage, and very responsive to treatment.
Protrusion
The inner material pushes against a weakened but still-contained outer wall, often pressing on a nerve.
Extrusion (herniation)
The outer wall tears and inner material escapes, commonly causing stronger nerve compression and sciatica.
Sequestration
A fragment separates from the disc. The most advanced stage — yet even these have responded to conservative care in published reports.

Favour anti-inflammatory foods: cooked vegetables, whole grains, ginger, turmeric, and healthy fats like ghee in moderation.
Stay well hydrated — spinal discs are largely water and depend on it to stay cushioned.

Avoid
Prolonged unbroken sitting, slouching, and soft, sagging mattresses or sofas.

Cold, dry, and processed foods, and skipping meals — all aggravate Vata.
High-impact activity, heavy gym lifts, and long bumpy travel during an acute flare.
Smoking, which reduces blood supply to the discs and slows healing.
Sleeping on your stomach, which strains the lower back.
Movement & Yoga — Gently, and Under Guidance
Gentle, appropriate movement strengthens the muscles that support the spine — but the wrong exercise at the wrong time can make a disc bulge worse. As a hospital, we don't recommend self-prescribing exercises from the internet, especially during an acute phase.
Walking on level ground is usually the safest way to stay active and keep circulation up.
Gentle, supervised yoga and stretching can help once the acute pain settles — but only postures cleared by your treating doctor for your specific disc level.
Avoid deep forward bends, heavy twists, and sit-ups while a disc is actively bulging — these raise pressure on the disc.
